• Fibrinolisis química asociada a Stent

            En muchos casos se requiere de la asociación de dos o más técnicas en el ictus; por esta razón, como mencioné anteriormente, el neurorradiólogo intervencionista debe conocer y manejar correctamente todas las técnicas de la terapéutica endovascular. En la (Fig 7) (Ver Fig 7 jpg) y (Fig8) (Ver Fig 8 jpg) podemos ver un ejemplo de ello. El primero es un  paciente de 69 años de edad, con antecedentes de AITs a repetición desde 7 días antes de su ingreso. Presentaba importantes cambios neurológicos asociados con la posición: al subir la cabeza por encima de los 30º, el paciente presentaba hemiplejía y disartria. Se inició heparinoterapia sistémica y fue trasladado a nuestro centro.

    En la valoración inicial, el estudio angiográfico mostró una estenosis severa del sifón carotídeo, observándose también la presencia de un trombo en la zona de la estenosis, y probablemente responsable de las variaciones neurológicas del paciente al colocarlo por encima de 30º (a). Se inició una terapia con UK, hasta alcanzar la dosis máxima de 1.200.000 UI de UK. En el control post UK podemos ver la resolución del trombo (b), persistiendo la zona de estenosis pre oclusiva (c y e), por lo que se realizó una angioplastia con stent balón expandible, obteniéndose una resolución completa (d y f). El cuadro (h) muestra el control de TAC a las 24 horas.

    En pacientes con contraindicación absoluta para la fibrinólisis IA, como es el caso que vemos a continuación, en ocasiones también es necesaria la combinación de diferentes técnicas para poder obtener una correcta recanalización: ictus agudo en una paciente en tratamiento anticoagulante por FA (Fig 8) (Ver Fig 8 jpg).

    Cuadro brusco de hemiplejia derecha y afasia, y que llegó a nuestro centro 90 minutos después del inicio del cuadro. En el estudio angiográfico inicial observamos una oclusión completa del segmento M1 de la arteria cerebral media izquierda (a). Navegamos un balón de angioplastia sobre una guía 0.014 (b), realizando una angioplastia intratrombo  (c). La figura (d) muestra el control post angioplastia. Se obtuvo una recanalización parcial de la arteria, con restos de trombo en M1. Navegamos sobre la guía con un stent Wingspan, procediendo a desplegarlo en el segmento M1, realizando una angioplastia intrastent (e), obteniéndose la recanalización completa de la arteria (f). En la figura (g) se observa el parenquimograma post tratamiento con una circulación normal, y la figura (h) nos muestra el TAC de control. La paciente regresó asintomática.
