    Tanto la intoxicación por fármacos como la retirada de alguno de ellos en concreto, son causas frecuentes de delirio en el paciente anciano polimedicado. Los fármacos con más probabilidad de producir este cuadro son los que tienen propiedades anticolinérgicas,  como pueden ser los fármacos anticatarrales, antihistamínicos y neurolépticos. Los  pacientes en tratamiento con prednisona también pueden experimentar psicosis por corticoides. Otros fármacos que guardan relación con el delirium son los hipnóticos, los opiáceos utilizados como analgésicos y los antiparkinsonianos. Los síndromes de abstinencia pueden estar causados por la retirada brusca de muchas sustancias como pueden ser las benzodiacepinas, los antidepresivos, en concreto, inhibidores selectivos de la recaptación de serotonina o el alcohol cuya suspensión da lugar al delirium tremens tras 72-96 horas. También se han relacionado con el delirium fármacos antiarritmicos como la  amiodarona, antiepilépticos como el levetiracetam , fármacos inmunosupresores  y citostáticos empleados como tratamiento quimioterápico. Por último, la exposición a ciertos metales pesados como oro, plomo, mercurio, talio, cinc, arsénico y bismuto pueden producir cuadros de delirium.
